摘要: BACKGROUND:Noninvasivemonitoringofintra-abdominalpressure(IAP)bymeasuringabdominalwalltension(AWT)waseffectiveandfeasibleinpreviouspostmortemandanimalstudies.ThisstudyaimedtoinvestigatethefeasibilityoftheAWTmethodfornoninvasivelymonitoringIAPintheintensivecareunit(ICU).METHODS:Inthisprospectivestudy,weobservedpatientswithdetainedurethralcathetersintheICUofShanghaiTenthPeoplesHospitalbetweenApril2011andMarch2013.ThecorrelationbetweenAWTandurinarybladderpressure(UBP)wasanalyzedbylinearregressionanalysis.TheeffectsofrespiratoryandbodypositiononAWTwereevaluatedusingthepairedsamplesttest,whereastheeffectsofgenderandbodymassindex(BMI)onbaselineAWT(IAP<12mmHg)wereassessedusingone-wayanalysisofvariance.RESULTS:Atotalof51patientswerestudied.AsignificantlinearcorrelationwasobservedbetweenAWTandUBP(R=0.986,P<0.01);theregressionequationwasY=–1.369+9.57X(P<0.01).Thereweresignificantdifferencesamongthedifferentrespiratoryphasesandbodypositions(P<0.01).However,genderandBMIhadnosignificanteffectsonbaselineAWT(P=0.457and0.313,respectively).CONCLUSIONS:TherewasasignificantlinearcorrelationbetweenAWTandUBPandrespiratoryphase,whereasbodypositionhadsignificanteffectsonAWTbutgenderandBMIdidnot.Therefore,AWTcouldserveasasimple,rapid,accurate,andimportantmethodtomonitorIAPincriticallyillpatients.
